Supportive Housing Case Manager (CNA/LVN)
Program Type: Mental Health — Supportive Housing Services
Schedule: Monday – Friday | 4:00 PM – 12:00 AM | Full-Time (40 hours/week)

POSITION OVERVIEW

We are seeking a motivated and compassionate Supportive Housing Case Manager (CNA/LVN) to join The Good Seed team in Los Angeles, CA . This position supports young adult males (ages 18–25) experiencing housing instability, mental health challenges, and/or substance use through a trauma-informed, client-centered approach.

The Supportive Housing Case Manager plays a key role in helping clients secure and maintain stable housing while building essential life skills and independence. This role focuses on empowering young adults through individualized support, resource connection, and coordinated care efforts to promote long-term stability and self-sufficiency.

KEY RESPONSIBILITIES

  • Conduct intake assessments and develop individualized housing stability and service plans

  • Update service plans quarterly or as client needs change

  • Assist clients in securing and maintaining permanent housing (lease signing, landlord mediation, rental negotiations, unit inspections)

  • Coordinate access to medical, mental health, substance use treatment, and supportive services

  • Support benefits enrollment (SSI/SSDI, SNAP, Medicaid) and employment resources

  • Utilize trauma-informed and harm-reduction approaches in all client interactions

  • Advocate for clients with landlords, courts, hospitals, and benefit agencies

  • Provide crisis intervention and de-escalation support as needed

  • Participate in case conferences, team meetings, and supervision

  • Collaborate with property managers, housing navigators, peer specialists, and clinical teams

  • Maintain accurate documentation, case notes, and client records

  • Manage a flexible caseload (approximately 12–18 clients)

  • Perform other duties as assigned

QUALIFICATIONS

Required:

  • Bachelor’s degree in Social Work, Psychology, Human Services , or a related field (or equivalent experience)

  • 1–2 years of experience in case management or social services

  • Ability to work with individuals experiencing homelessness, mental illness, substance use, and/or trauma

  • Strong organizational, documentation, and time-management skills

  • Proficiency in Microsoft Office and case management systems

  • Ability to work independently and manage a flexible schedule

  • Valid driver’s license and willingness to travel locally as needed

  • Ability to pass background checks

Preferred:

  • CNA or LVN certification

  • Knowledge of Housing First, harm reduction, trauma-informed care, and motivational interviewing

  • Experience working with young adults or supportive housing programs

  • Familiarity with Los Angeles County housing and social service systems

  • CPR/First Aid certification

WORK ENVIRONMENT

This position operates in community-based and office settings , including client homes, program sites, and partner locations across Los Angeles, CA. The role requires frequent local travel and working in dynamic environments.

Candidates must be able to remain calm and effective in challenging situations while consistently demonstrating professionalism, respect, and a commitment to client-centered care. The role may involve standing and walking for extended periods, occasional lifting (up to 50 lbs), and navigating physically and emotionally demanding situations.
