Social Worker (LCSW)

Day Shift Sun - Thurs. 40hrs week, 8 hrs a day, can start between 8 and 9 am.. half hour lunch. Must have experience in social work, have an MSW standard for CA. Have acute care experience and active license. Must start ASAP. Need a true Experience in acute care/ED/homeless preferred.

Position Overview

The LCSW will assess patients’ psychosocial needs, coordinate discharge planning, and provide crisis intervention and counseling services.

Responsibilities

• Conduct psychosocial assessments

• Develop safe and effective discharge plans

• Coordinate with case management, nursing, and providers

• Provide counseling to patients and families

• Facilitate referrals to community resources

• Maintain accurate documentation

Qualifications

• Active California LCSW license required

• Acute care hospital experience preferred

• Strong discharge planning experience

• Knowledge of insurance and community resources
