Position Summary
We are hiring a Transitional Care Manager to lead referral development, community outreach, and relationship-building efforts across both of our Chicagoland branches. This is a dynamic, externally-facing role that sits at the intersection of healthcare marketing and care coordination — ideal for a healthcare professional who is equally comfortable walking hospital floors, presenting to physicians, and building long-term referral partnerships.
You will be our face in the field, leveraging our 4.5- and 5-star ratings, dual-branch capabilities, and strong clinical outcomes to grow our referral pipeline from hospitals, skilled nursing facilities, physician offices, and community organizations.
This position works closely with the Administrator and Operations leadership team and plays a direct role in agency growth, patient census, and market expansion.
Key Responsibilities
Referral Development & Business Growth
Proactively identify, cultivate, and grow referral relationships with hospitals, SNFs, ALFs, physician offices, discharge planners, case managers, and community organizations
Conduct regular in-person visits to referral sources to build trust, present agency capabilities, and position Safe Life as the preferred home health provider in the market
Leverage Safe Life's 4.5-star (Lombard) and 5-star (Chicago) CMS ratings as key differentiators in all outreach and sales conversations
Develop and execute a territory-based outreach strategy targeting high-volume referral sources across both branch service areas
Track all referral activity, leads, conversions, and relationship status in a CRM or referral tracking system; report outcomes to leadership weekly
Identify gaps in referral source coverage and develop targeted action plans to fill them
Transitional Care Coordination
Serve as the primary liaison between Safe Life and referral source discharge teams during patient transitions from hospital or facility to home
Facilitate smooth, timely patient transitions by coordinating with clinical staff, intake, and scheduling to ensure seamless care continuity
Conduct in-person or virtual introductions with patients and families at the point of discharge to build confidence in Safe Life's services and support enrollment
Communicate patient status updates, care plan summaries, and follow-up outcomes back to referring providers to strengthen the referral loop
Act as a resource for referral partners on home health eligibility, covered services, and the intake and onboarding process
Community Outreach & Marketing
Represent Safe Life Home Health Care at community health fairs, networking events, hospital forums, senior expos, and professional association meetings
Build and maintain relationships with community stakeholders including senior centers, faith-based organizations, social workers, and elder law attorneys
Collaborate with leadership on marketing materials, digital presence, and agency branding initiatives
Identify sponsorship and co-marketing opportunities with aligned community partners
Reporting & Strategy
Provide weekly and monthly referral activity reports to the Administrator and Operations team, including pipeline status, conversion rates, and partnership growth
Analyze referral trends and market data to inform territory strategy and identify new growth opportunities
Participate in leadership and operational meetings to align marketing efforts with agency capacity and clinical goals
Qualifications
Minimum 3 years of experience in healthcare marketing, business development, transitional care, case management, or a related field — required
Background in home health, hospice, SNF, hospital discharge planning, or post-acute care strongly preferred
Demonstrated track record of building referral relationships and growing patient census in a healthcare setting
Strong understanding of home health Medicare eligibility, the discharge planning process, and post-acute care coordination
Exceptional interpersonal, communication, and presentation skills — comfortable engaging C-suite executives, physicians, discharge planners, and families alike
Self-motivated, goal-oriented, and able to work independently in the field with minimal supervision
Proficient in Microsoft Office Suite and CRM or referral tracking tools
Valid Illinois driver's license and reliable transportation required for field-based outreach
Clinical background (RN, LPN, MSW, or similar) is a strong plus but not required
