Job Title: Social Worker II
Location: Oklahoma City, OK 73104
Pay: $1600 - $1800/weekly
Essential Responsibilities
- Provide social services to patients and their families for an assigned medical service.
- Conduct discharge planning evaluations to gather essential information.
- Perform psychosocial assessments and communicate findings with hospital staff.
- Counsel patients and families regarding medical conditions, treatment, financial, or emotional challenges.
- Provide crisis intervention for high-risk cases including child/elder abuse, neglect, and domestic violence.
- Assist patients and families with adjustments to illness, treatment, or changes in functional status.
- Coordinate referrals and community resources to support continuity of care.
- Serve as a patient advocate for ethical dilemmas, informed decision-making, and end-of-life care.
- Assist in post-hospital placement or transfer to post-acute care facilities.
- May supervise students or newly hired employees.
- Perform other duties as assigned.
Qualifications
Education:
- Master’s Degree in Social Work from an accredited school (required).
Licensure/Certification:
- LMSW strongly preferred.
Experience:
- Previous hospital or healthcare social work experience preferred.
Knowledge, Skills & Abilities:
- Strong knowledge of social work principles, casework practices, and community resources.
- Excellent interpersonal, counseling, and crisis intervention skills.
- Ability to analyze situations and determine effective courses of action.
- Strong communication skills to explain technical or sensitive information to patients, families, and staff.
