Key Responsibilities: Initiate discharge plans; conduct follow-ups; coordinate referrals (housing, outpatient, medical, recovery support); resolve barriers (housing, employment, legal, financial); maintain documentation; ensure releases/referrals are completed; communicate professionally; collaborate with teams.
