Clinical & Community Liaison – Home Care & Infusion Services

Position Overview

We are seeking an experienced Clinical & Community Liaison to drive referral growth and build strong relationships across North Dallas, Plano, Frisco, and McKinney.

This role requires a professional who understands both the clinical and community aspects of healthcare marketing and can effectively represent services to hospitals, facilities, physicians, and community partners.

The ideal candidate has experience marketing home care, private duty nursing, and clinical services, and is comfortable working in a field-based, relationship-driven role.

Key ResponsibilitiesBusiness Development & Relationship Management

• Develop and maintain referral relationships with hospitals, discharge planners, rehabilitation facilities, skilled nursing facilities, assisted living and memory care communities, physicians, and community partners
• Conduct regular in-person visits to referral sources and facilities
• Represent the organization at networking events and community engagements

Clinical Liaison Responsibilities

• Present and position services including private duty nursing, infusion therapy, wound care, and post-discharge support
• Identify appropriate patient referrals based on clinical needs
• Collaborate with internal clinical teams to ensure appropriate case placement

Community Liaison Responsibilities

• Build relationships within local communities and senior living environments
• Educate families and referral partners on available services
• Support outreach initiatives and community engagement strategies

Marketing Strategy & Execution

• Develop and execute territory-based marketing plans
• Track referral sources, conversions, and growth opportunities
• Provide insights on market trends, competition, and service demand
• Collaborate with leadership to refine outreach strategies

Qualifications

• Minimum 2–3 years of experience in healthcare marketing or business development
• Experience as a Clinical Liaison and/or Community Liaison (required)
• Experience marketing home care, private duty nursing, or clinical services
• Strong understanding of referral networks and post-acute care landscape
• Excellent communication, presentation, and relationship-building skills
• Self-motivated and able to work independently in a field-based role

Preferred Qualifications

• Existing relationships within North Dallas, Plano, Frisco, or McKinney
• Experience working with hospitals, SNFs, assisted living, or home health agencies
• Understanding of care coordination and post-discharge patient needs

• Flexible schedule with regular in-person visits required

Compensation

• Base salary: $60,000 – $65,000 annually (based on experience)
• Performance-based incentives:
• Home Care: Commission based on billed hours generated from new clients
• Skilled Nursing / Clinical Services: Per admission bonus
• Total compensation potential: $70,000 – $100,000+ based on performance
