Advanced Practice Provider-Full Time Community Behavioral Health & Justice Programs

  • Salary Range: $ 113,506 to 170,269/annually (full-time equivalent: based experience)  
  • Location: Larimer county, Colorado
  • Status: Full-Time and Part-Time Opportunities Available

Role Overview:

SummitStone Health Partners is seeking an Advanced Practice Provider to support both our acute care behavioral health services and specialized community-based forensic programs, including:

  •  Alternatives to Incarceration for Individuals with Mental Health Needs (AIIM) 
  • 8th Judicial District Wellness Court 
  • Residential Dual Diagnosis Treatment (RDDT) 

This role bridges behavioral health care, residential treatment, and the criminal justice system, providing psychiatric evaluation, medication management, and collaborative care to individuals with serious mental illness and co-occurring substance use disorders.

The APP will work closely with multidisciplinary teams, court systems, and community partners to support recovery, reduce recidivism, and improve public safety through treatment-centered alternatives to incarceration and successful community reintegration.

 Essential Duties:

  • Provide psychiatric evaluation, diagnosis, and medication management across acute, residential, and community-based forensic settings 
  • Serve as a core clinical member of multidisciplinary teams, including therapists, case managers, residential staff, probation officers, and court representatives 
  • Develop and support integrated treatment plans addressing mental health, substance use, and criminogenic needs 
  • Collaborate with:                
    • Court systems (e.g., Wellness Court participation and reporting)
    • Community supervision and case management teams
    • Residential treatment staff (RDDT) 
  • Support care transitions between acute care, residential treatment, community-based and outpatient services
  • Provide care aligned with Medications for Addiction Treatment (MAT) and harm reduction principles 
  • Monitor treatment adherence, response to medications, and overall clinical stability 
  • Maintain timely, accurate, and compliant documentation 
  • Ensure adherence to all local, state, and federal healthcare regulations

Minimum Qualifications

  • Graduate degree as a Nurse Practitioner (APN) or Physician Assistant (PA) from an accredited program
  • Active Colorado licensure (or ability to obtain)
  •  Experience or strong interest in working with justice-involved and high-acuity behavioral health populations 
  • Commitment to serving diverse, underserved, and marginalized communities 
  • Knowledge of or willingness to learn MAT and harm reduction approaches

Preferred Qualifications

  • 2+ years of experience in behavioral health and/or substance use treatment
  • Experience in forensic, residential, or community mental health settings
  • Familiarity with criminal justice systems, problem-solving courts, or offender assessment processes 
  • Experience working in residential dual diagnosis or reentry programs
  • Spanish or other bilingual skills
  • Knowledge of withdrawal management and detox protocols
