Senior Field RN Care Manager Highlights

Monday–Friday schedule (8:30am – 5:00pm)
Field-based role conducting 2–4 UAS assessments daily
Must reside in Albany or Rensselaer County, NY
Permanent, direct hire opportunity

Senior Field RN Care Manager Responsibilities

Conduct UAS assessments and reassessments for LTSS members
Develop, implement, and manage individualized care plans
Coordinate care across providers, caregivers, and community resources
Monitor member progress and adjust care plans as needed
Educate members and families on services, benefits, and care options
Collaborate with interdisciplinary care teams to ensure quality outcomes
Maintain accurate documentation in compliance with state and federal guidelines
Identify high-risk members and support appropriate level-of-care transitions

Senior Field RN Care Manager Requirements

Active New York RN license
4+ years of RN experience
UAS assessment experience required
Ability to perform field-based home/site visits
Strong care management and coordination experience
Valid driver’s license (personal transportation preferred)

Preferred Qualifications

Bachelor’s degree in Nursing (BSN)
Experience in Managed Care / MLTC / LTSS
Background working with high-acuity populations
Knowledge of community resources and care coordination
