Social Worker-GREAT PAY -KOREAN SPEAKING -ADULT DAY HEALTH CENTER-IRVINE

Job Summary:

We are seeking a qualified and experienced social worker to join our Adult Day Health Care (ADHC) facility in Irvine , CA. The ADHC Social Worker will be responsible for providing psychosocial assessments and developing care plans for our participants, as well as coordinating with our interdisciplinary team to ensure the highest quality of care.

Responsibilities:

• Conduct initial psychosocial assessments and ongoing assessments for all participants, identifying strengths and areas of need.
• Develop individualized care plans that address participants' physical, emotional, and social needs in collaboration with the interdisciplinary team.
• Provide counseling, supportive services, and referrals to participants and their families as needed.
• Develop and maintain relationships with community resources and service providers to ensure participants receive appropriate services.
• Participate in care conferences, team meetings, and other interdisciplinary activities.
• Document all services provided in participants' medical records and maintain confidentiality of all participant information.
• Perform other duties as assigned by the Program Director or Clinical Manager.

Requirements:

• B.S degree in social work or a related field from an accredited institution.
• Minimum of one years of experience in social work, preferably in a healthcare setting.
• Knowledge of psychosocial assessment tools, care planning, and case management.
• Strong communication, interpersonal, and organizational skills.
• Ability to work independently and as a member of an interdisciplinary team.
• Bilingual in English and KOREAN is required.
