- · Oversee and coordinate all patient referral and intake operations.
- · Build & maintain strong relationships with hospitals, physicians, & community partners.
- · Ensure patients are admitted quickly, accurately, and in compliance with regulations.
- · Manage insurance verification, authorizations, and payer communication.
- · Collaborate with clinical teams to ensure seamless care transitions.
- · Track referral trends and contribute to strategic growth initiatives.
- · Support quality improvement efforts and enhance patient experience.
- · Serve as a trusted resource for patients, families, and referral sources.
- · 3–5+ years of healthcare experience (hospice or home health strongly preferred).
- · Strong leadership, organization, and problem-solving skills.
- · Experience with intake, admissions, or referral coordination.
- · Ability to build relationships and represent our mission in the community.
- · Knowledge of insurance processes, authorizations, and healthcare regulations.
