That’s where you come in.We’re looking for an Intake Coordinator for our Victoria, TX agency, who thrives at the intersection of operations, relationships, and purpose-driven work, someone who can lead the front door of our agency with both precision and heart.Why This Role MattersAs our Intake Coordinator, you are often the first impression of Crown Hospice—for patients, families, and referral partners. You ensure that every patient’s transition into hospice care is smooth, timely, and handled with compassion, while also driving the operational excellence behind the scenes.What You’ll Do
  • · Oversee and coordinate all patient referral and intake operations.
  • · Build & maintain strong relationships with hospitals, physicians, & community partners.
  • · Ensure patients are admitted quickly, accurately, and in compliance with regulations.
  • · Manage insurance verification, authorizations, and payer communication.
  • · Collaborate with clinical teams to ensure seamless care transitions.
  • · Track referral trends and contribute to strategic growth initiatives.
  • · Support quality improvement efforts and enhance patient experience.
  • · Serve as a trusted resource for patients, families, and referral sources.
What We’re Looking For
  • · 3–5+ years of healthcare experience (hospice or home health strongly preferred).
  • · Strong leadership, organization, and problem-solving skills.
  • · Experience with intake, admissions, or referral coordination.
  • · Ability to build relationships and represent our mission in the community.
  • · Knowledge of insurance processes, authorizations, and healthcare regulations.
  • · Confident communicator with a professional, compassionate presence.
  • · Someone who is both detail-oriented and big-picture focused.
