Job Title: Clinic Care Coordinator Registered Nurse - Fort Defiance,Arizona

We are seeking a dedicated Care Coordinator RN for our Nahata Dziil Clinic in Sanders, AZ. As a Care Coordinator, you will work collaboratively with our support nursing staff and providers to deliver top-quality patient care.

Responsibilities:

Collaborate with interdisciplinary teams to develop patient care plans
Monitor and coordinate patient care activities
Educate patients and families on disease management, treatment plans, and medication regimens
Provide ongoing support and resources to patients and their families
Participate in quality improvement initiatives
Requirements:

Active RN license in the state of Arizona
Minimum of 2 years of experience in case management, care coordination or related role
Strong communication and interpersonal skills
Ability to multitask and prioritize tasks effectively
Must be willing to work weekdays, with no weekends or holidays
