LTSS Service Care Manager

Title - LTSS Service Care Manager
Duration - 3 months | Possible extension and conversion
Shift - Mon to Fri | 8 am to 5 pm
Location - Austin TX (3 day member visits, 2 days remote)
Pay rate: $26-32.00/hr.

Day-to-Day Responsibilities

• Conduct member visits (3 days per week) and complete documentation tasks (2 days per week)
• Manage an assigned caseload and prioritize daily tasks effectively
• Perform H2060 assessments in accordance with program guidelines
• Complete and submit all required assessment documentation and forms
• Make follow-up calls to members to ensure continuity of care and address concerns

Required Qualifications

• Bachelor’s Degree (required)
• Valid Driver’s License (required)
• 2+ years of experience in:
• Managed care environments OR Supporting individuals with disabilities and vulnerable populations with chronic or complex conditions

Preferred Qualifications

• Experience in Home Health or Nursing
• Strong understanding of care coordination and healthcare systems
