Title - LTSS Service Care Manager
Duration - 3 months | Possible extension and conversion
Shift - Mon to Fri | 8 am to 5 pm
Location - Austin TX (3 day member visits, 2 days remote)
Pay rate: $26-32.00/hr.
Day-to-Day Responsibilities
- Conduct member visits (3 days per week) and complete documentation tasks (2 days per week)
- Manage an assigned caseload and prioritize daily tasks effectively
- Perform H2060 assessments in accordance with program guidelines
- Complete and submit all required assessment documentation and forms
- Make follow-up calls to members to ensure continuity of care and address concerns
Required Qualifications
- Bachelor’s Degree (required)
- Valid Driver’s License (required)
- 2+ years of experience in:
- Managed care environments OR Supporting individuals with disabilities and vulnerable populations with chronic or complex conditions
Preferred Qualifications
- Experience in Home Health or Nursing
- Strong understanding of care coordination and healthcare systems
