Job Summary:

As a Care Coordinator, you will play a pivotal role in providing comprehensive support and guidance to individuals living with dementia and their caregivers. Your primary focus will be on ensuring the delivery of personalized care and assistance to enhance the quality of life for those affected by dementia. Your compassionate approach, strong communication skills, and ability to navigate healthcare systems will be essential in facilitating access to resources and services tailored to the unique needs of each individual and their family.

Duties and Responsibilities:

  • Conduct comprehensive assessments of individuals living with dementia to identify their care needs, preferences, and goals.
  • Collaborate with clients, their families, and healthcare providers to develop personalized care plans that address physical, emotional, and social needs.
  • Regularly review and update care plans based on changes in the client's condition and evolving care needs.
  • Serve as a primary point of contact for clients and their families, providing information, guidance, and referrals to community resources and support services.
  • Coordinate access to healthcare services, including medical care, home health services, and specialized dementia programs.
  • Advocate for clients to ensure they receive timely and appropriate support from healthcare providers and community organizations.
  • Facilitate support groups, workshops, and educational sessions to promote knowledge sharing, peer support, and skill-building among caregivers.
  • Respond to crises and emergencies, such as sudden changes in the client's condition or caregiver burnout, by providing immediate support and coordinating appropriate interventions.
  • Collaborate with healthcare professionals, including physicians, social workers, and mental health specialists, to address complex care needs and ensure continuity of care.
  • Maintain accurate and up-to-date records of client assessments, care plans, interventions, and outcomes in accordance with regulatory requirements and organizational standards.
  • Other duties as assigned.

Background, Licensing Requirements:

Regulatory Requirements

  • High school diploma or equivalent degree required. Bachelor’s degree in nursing, social work, psychology or related field preferred.

Language Skills

  • Must possess good oral and written communication skills in the English language.

Skills

  • Good overall clinical skills, ability to work productively and effectively.
  • Ability to function well as a member of a multidisciplinary team.
