Client Care Manager

Overview

1Heart Caregiver Services is a fast-growing home care agency seeking a Client Care Manager who can manage client care while also driving growth through marketing and relationship building.

This role is perfect for someone who is both people-oriented and business-minded—someone who can ensure excellent care while actively bringing in new clients and building strong referral networks.

Key Responsibilities

Client Care & Operations

• Manage and oversee client care plans and services
• Conduct client intakes, assessments, and follow-ups
• Coordinate caregiver scheduling and coverage
• Ensure high client satisfaction and quality of care
• Handle urgent staffing or client needs

Marketing & Business Development (CORE FOCUS)

• Visit assisted living facilities, hospitals, rehab centers, and community partners
• Build and maintain strong referral relationships
• Generate new client leads through outreach and networking
• Represent the company in meetings, events, and facility visits
• Follow up on leads and convert them into active clients
• Maintain ongoing relationships to increase referrals

Qualifications

• Experience in home care, healthcare, or sales/marketing preferred
• Strong communication, networking, and relationship-building skills
• Confident meeting with facilities and professionals in person
• Highly organized and able to multitask
• Self-motivated with a results-driven mindset
• Bilingual (Spanish or Tagalog) preferred

Compensation & Growth

• $2,800 – $3,100 monthly base
• Commission based on new clients and growth

Schedule

• Full-time
• Flexible schedule with field visits and office work
• Occasional after-hours availability as needed
