Social Worker – Pediatric Subacute (BSW Level)

We are seeking a Bachelor-level Social Worker (BSW) to support children and families through care coordination, discharge planning, and psychosocial support. This position combines meaningful case management responsibilities with traditional social work services in a close-knit, multidisciplinary team setting.

Key Responsibilities:
Provide support to the primary social worker.
Conduct psychosocial assessments and develop care plans.
Provide individual and family support, including crisis intervention.
Coordinate discharge planning, referrals, and community resources.
Collaborate with nurses, physicians, and insurance representatives.
Support treatment team meetings and document care planning.
Assist with insurance authorizations and follow up.
Qualifications:
Bachelor’s degree in Social Work (BSW required; MSW welcome)
1+ year experience in a healthcare, pediatric, or case management setting preferred
Knowledge of child development, family systems, and community resources
Excellent communication, teamwork, and documentation skills
Familiarity with discharge planning and interdisciplinary collaboration is a plus

WORKING ENVIRONMENT
While performing the duties of this job, the employee occasionally works near moving mechanical parts and is occasionally exposed to toxic or caustic chemicals, risk of radiation and vibration.
The noise level in the work environment is usually moderate.
The employee must be able to work near other co-employees and concentrate efforts on tasks despite noise, interruptions, and general commotion.
