Position: Supportive Housing Case Manager (United for Youth)

Position Summary
The Supportive Housing Case Manager provides housing navigation and intensive case management services to current and former foster youth ages 18–28 enrolled in the United for Youth (U4Y) program. The program prioritizes pregnant and parenting transition-age foster youth and young families experiencing housing instability.
Under the supervision of the Program Manager, this role supports youth in securing and maintaining safe, stable housing while addressing barriers through individualized service planning, resource coordination, and ongoing stabilization support. Services are delivered using youth-centered, trauma-informed, family-centered, and Housing First approaches.
Key Responsibilities
- Identify and coordinate housing opportunities, including bridge housing, shared housing, masterlease units, and independent rentals.
- Conduct housing assessments and develop individualized housing stability plans.
- Support housing search activities, applications, documentation, budgeting, and movein readiness.
- Serve as liaison between youth, landlords, and property managers to facilitate placement, tenancy support, and conflict resolution.
- Monitor housing retention and address barriers through followup, mediation, and advocacy.
- Process and manage rental assistance documentation, including agreements, payment requests, and supporting materials.
- Cultivate and maintain relationships with landlords and housing providers to expand available resources.
Case Management and Youth Support
- Conduct comprehensive intakes, assessments, and individualized case planning.
- Develop and monitor service plans addressing housing stability, education, employment, parenting, life skills, and overall wellbeing.
- Provide strengthsbased coaching, crisis intervention, and problemsolving support.
- Connect youth to internal and communitybased resources, including workforce development, education, mentoring, benefits, mental health services, prenatal and pediatric care, and early childhood supports.
- Support the development of independent living and parenting skills, including budgeting, tenancy responsibilities, and child safety.
- Maintain consistent contact to support goal progress and longterm stability.
Collaborative Coordination
- Coordinate services with United for Youth partner agencies to ensure wraparound support.
- Participate in case conferencing, crossagency meetings, and collaborative planning.
- Maintain timely communication with partners regarding participant progress and service needs.
Documentation, Reporting, and Compliance
- Maintain accurate, timely, and complete case records and housing documentation in required databases.
- Track placements, services delivered, and participant outcomes in alignment with program targets.
- Contribute data and documentation for reporting, evaluation, and grant compliance.
- Ensure confidentiality and ethical handling of participant information.
Outreach and Engagement
- Conduct outreach to eligible transitionage foster youth and referral partners.
- Educate participants on tenant rights and responsibilities, parenting resources, and community services.
- Engage youth using culturally responsive, traumainformed, and motivational approaches.
Administrative and Team Participation
- Participate in supervision, team meetings, trainings, and collaborative convenings.
- Maintain a flexible schedule, including field visits and occasional evening availability as needed.
- Escalate complex cases or safety concerns to the Program Manager.
- Perform other duties as assigned.
Qualifications
Education
Bachelor’s degree in Social Work, Human Services, Psychology, or a related field required. Relevant experience may substitute for education.
Experience
- Experience in housing navigation, case management, or services for transitionage youth, foster youth, or homeless or atrisk populations required.
- Experience working with pregnant or parenting youth preferred.
- Experience collaborating with landlords, housing programs, or rental assistance initiatives preferred.
Knowledge, Skills, and Abilities
- Knowledge of housing resources, landlordtenant practices, and community service systems in San Diego County.
- Ability to engage young adults with complex needs using traumainformed and culturally responsive approaches.
- Strong organizational, documentation, and timemanagement skills.
- Ability to work independently in community settings and collaboratively within a multidisciplinary team.
- Demonstrated cultural humility and respect for diverse populations.
Preferred Language Capacity
Bilingual in English and Spanish or another language reflective of the populations served preferred.
Additional Requirements
- Valid California driver’s license, acceptable driving record, current automobile insurance, and reliable transportation.
- Ability to travel throughout San Diego County.
Physical Requirements
- Ability to sit for extended periods while completing documentation and attending meetings.
- Frequent travel by car to meet with participants and attend agency or community meetings.
- Ability to perform essential job functions with or without reasonable accommodation.

- Combination of fieldbased work and shared office environment.
- Use of standard office equipment.
