Job Summary:
The LVN Clinical Intake Coordinator is responsible for managing incoming referrals, verifying insurance coverage, and coordinating communication between patients, families, physicians, and internal healthcare teams. This role supports home health operations by ensuring smooth intake processing and scheduling.
Key Responsibilities:
- Coordinate incoming referrals and verify insurance coverage
- Handle calls from patients, families, physicians, and care teams
- Manage patient intake, scheduling, and follow-ups
- Track hospitalizations and ensure proper referral continuity
- Communicate updates with nursing, therapy, and support staff
- Assist with staffing and scheduling coordination
- Maintain accurate documentation and records
Required Qualifications:
- LVN License (Mandatory)- Licensed Vocational Nurse
- Minimum 1 year of healthcare experience
- Strong communication and coordination skills
- Experience with patient intake / scheduling preferred
Education:
- High School Diploma / GED – Required
- Associate / Technical Degree – Preferred
Preferred:
- 3+ years healthcare experience
- RN or LVN licensure
