Compassionate Home Care Coordinator needed for Seniors in Southern California – Support, Dignity, and Quality Care Where It Matters Most

Location: San Diego, CA

Pay range: $22.39 - $28.09

Job Overview:

Under the supervision of the PACE Home Care Coordination RN Manager, the Home Care Coordinator (HHA) supports the Interdisciplinary Team (IDT) by coordinating key home-based services for San Diego PACE participants. Responsibilities include conducting home safety evaluations and arranging IDT-approved services such as meal delivery, caregiver support, and durable medical equipment (DME).

This role serves as a compassionate advocate for participants, ensuring care is delivered in a person-centered way that aligns with their goals. The coordinator also collaborates with families, care agencies, and vendors to manage schedules, monitor service quality, and recommend updates to care plans as needed.

Key Responsibilities:

  • Coordinate delivery of IDT-approved services, including meals, caregiver support, and durable medical equipment (DME).
  • Conduct home safety evaluations during enrollment, semi-annually, and as needed for address changes or participant status updates.
  • Complete and maintain Home Care Coordination care plans in collaboration with the Interdisciplinary Team (IDT).
  • Document and track service delivery in electronic medical records (EMR), ensuring compliance with clinical and regulatory standards.
  • Communicate regularly with participants, families, and care providers to ensure person-centered, coordinated care.
  • Support homebound and RCFE participants through home visits, supply deliveries, and monthly service checks.

Requirements


Requirements to apply:


  • Active CNA and California Home Health Aide (HHA) certifications required
  • CPR and First Aid certification from the American Heart Association
  • At least 1 year of experience working with elderly or frail individuals
  • Strong understanding of HHA scope of practice; works within defined limits
  • Effective team player with clear communication skills
  • Valid California driver’s license required
