Faith and Hope Hospice & Palliative Care was established in 2007 and is accredited by The Joint Commission (TJC). We are dedicated to providing compassionate end-of-life care with dignity, supported by a team of caring professionals committed to our patients' well-being.


We are seeking a compassionate and organized Care Coordinator to join our hospice team. This role is essential in ensuring seamless care delivery to patients and families navigating end-of-life journeys. As the central point of contact, you will manage admissions, scheduling, and communication across our interdisciplinary team (IDG) while providing support to patients and their families.

  • Coordinate hospice services according to patient care plans.
  • Support patients and families, providing education and guidance on hospice care.
  • Facilitate communication between the IDG and assigned patients.
  • Monitor patient conditions and coordinate management of symptoms, including pain.
  • Prepare patient care reports and participate in care plan meetings.
  • Act as a physician agent for prescription requests following agency protocols.
  • Reorder patient medications and maintain supply inventory.
  • Ensure compliance with all hospice policies, procedures, and regulatory requirements.
  • Participate in departmental meetings, QAPI processes, and required training.
  • Maintain confidentiality and professionalism in all interactions.

  • Graduate of an accredited School of Nursing with a current LVN license in California.
  • Minimum 1 year of nursing experience in an acute care setting.
  • Current CPR Certification (or completion within 6 months of hire).
  • 2+ years of hospice or home health experience preferred.
  • Knowledge of local, state, and federal hospice regulations.
  • Strong communication, organizational, and interpersonal skills.
  • Proficiency with computer systems and relevant software.
  • Ability to manage stress, remain flexible, and maintain confidentiality.

Pay: $32.00 - $35.00 per hour
