A well-established healthcare organization serving the greater San Diego area with over 20 clinic and program sites. The Program of All-Inclusive Care for the Elderly (PACE) provides comprehensive, coordinated care designed to help seniors maintain independence and improve overall quality of life.
Position Overview
We are seeking a PACE Home Care Coordinator (HHA/CNA) to join a dedicated care team in a full-time, direct hire role. This position works under the supervision of the PACE Care Coordination Manager to support home care services and ensure high-quality, coordinated care for program participants.
Key Responsibilities
- Conduct home safety evaluations (pre-enrollment, annual, semi-annual, and change of address)
- Coordinate and monitor IDT-approved services for PACE participants
- Arrange meal deliveries, durable medical equipment, and incontinence supplies
- Coordinate appropriate caregiver services
- Develop and maintain Home Care Coordination care plans
- Document and track services, actions, and delivery status in EMR systems
- Ensure compliance with DHCS and CMS regulations and guidelines
- Communicate regularly with participants, families, and healthcare providers to ensure continuity of care
Requirements
- Active CNA certification
- Active California Home Health Aide (HHA) certification
- CPR and First Aid certification from the American Heart Association
- Minimum 1 year of experience working with frail or elderly populations
- Valid California Driver’s License
Compensation & Benefits
- $22.39-$28.09/hr DOE
