We are seeking a dedicated and experienced Geriatric Care Manager to provide comprehensive support and coordination for elderly home based clients. The ideal candidate will possess a strong background in patient assessment, discharge planning, and post-acute care, with expertise in managing medical needs of older adults. As a Geriatric Care Manager, you will serve as a vital link between patients, families, and healthcare providers to ensure personalized, high-quality care that promotes independence and well-being.
Responsibilities
- Conduct thorough patient assessments, including vital signs, medical history review, and functional evaluations tailored to geriatric populations.
- Develop individualized care plans that address medical, behavioral health, and social needs of elderly patients.
- Coordinate multidisciplinary services across inpatient, outpatient, and post-acute care settings to facilitate smooth transitions and continuity of care.
- Manage discharge planning processes by collaborating with hospital staff, community resources, and family members to ensure appropriate follow-up care.
- Monitor patient progress through diagnostic evaluations, case management activities.
- Maintain detailed medical records in compliance with HIPAA regulations while ensuring accurate documentation of patient care activities.
- Provide education on health maintenance, medication management, and preventive strategies specific to disability and geriatrics.
Experience
- Experience working with geriatric populations in home care or similar
- Proven background in managing complex cases
- Strong knowledge of physiology related to aging, medical terminology
- Demonstrated expertise in discharge planning, post-acute care coordination
- Ability to assess vital signs accurately and comprehensive patient assessments.
