The Opportunity

We're seeking an experienced Aging Life Care Professional to join our growing team.

This role is for someone who sees beyond tasks and protocols—someone who

recognizes the deep privilege of serving families during vulnerable seasons. You'll

provide comprehensive care management, advocacy, and coordination for older

adults and their families throughout Metro Atlanta.

Position Details

This is a 1099 independent contractor position with hourly compensation based on

experience and credentials. This arrangement offers flexibility while allowing you to

build a meaningful practice serving Metro Atlanta's aging community.

What You'll Do

  • Conduct holistic assessments of clients' physical, emotional, social, and
  • environmental needs
  • Develop and implement personalized care plans that honor each client's
  • dignity and preferences
  • Coordinate services across healthcare providers, home care agencies, and
  • community resources
  • Advocate for clients in medical settings, ensuring their voices are heard
  • Provide crisis intervention and support during transitions (hospitalizations,
  • moves, care level changes)
  • Offer education and guidance to family caregivers navigating difficult
  • decisions
  • Monitor and adjust care plans as needs evolve
  • Maintain thorough documentation and communication with families and care
  • teams

Required Qualifications

  • Current RN license (Georgia) or LMSW/LCSW license (Georgia)
  • Minimum 2+ years experience in geriatric care, case management, or related
  • field
  • Knowledge of Medicare, Medicaid, and community aging services
  • Strong assessment, care planning, and coordination skills
  • Valid driver's license and reliable transportation for client visits throughout
  • Metro Atlanta
  • Proficiency with technology and documentation systems

Preferred Qualifications

  • Certified Care Manager (CMC & CCM) or Advanced Social Work Case Manager (C-ASWCM) certificationor interest in pursuing certification
  • Experience with Alzheimer's disease and dementia care
  • Background in hospital or skilled nursing settings
  • Established relationships with Metro Atlanta healthcare and aging services providers
  • Bilingual capabilities
