Job Title: Care Coordinator/ Supervisor
Location: Elmwood Park, IL (Office & Field Visits)
Employment Type: Full-Time
Compensation: $45,000 annually + Bonus

At Senior Helpers of Elmwood Park, we are passionate about improving the lives of seniors and their families. We are seeking a Care Coordinator to join our team and play a dual role in coordinating exceptional home care services and supporting our dedicated caregiver staff.

Position Summary:

The Care Coordinator ensures high-quality service delivery by coordinating caregiver schedules, supporting client relationships, and supervising field staff. This individual will serve as both a client service point of contact and a caregiver supervisor, ensuring alignment between client needs and caregiver performance. The ideal candidate is compassionate, organized, detail-oriented, self-starter who thrives in a fast-paced, mission-driven environment.

Key Responsibilities:

Client Service Coordination

  • Serve as the main point of contact for assigned clients and families
  • Schedule caregivers based on client needs, preferences, and staff availability
  • Conduct re-assessments, update care plans, and ensure services match care goals
  • Perform regular follow-up calls to monitor satisfaction and care outcomes
  • Address scheduling issues, caregiver call-offs, and client concerns with professionalism and urgency
  • Document all client communications, service changes, and condition updates accurately

Caregiver Supervision

  • Provide oversight and mentorship to caregivers to ensure quality care and compliance
  • Conduct field visits to observe caregiver performance and provide real-time coaching
  • Assist with ongoing training of caregivers as part of orientation and field visits
  • Ensure caregiver adherence to agency policies, safety protocols, and care plans
  • Support compliance with IDPH and IDoA standards through routine check-ins and documentation

Qualifications:

  • High School Diploma or GED required; Associate’s or Bachelor’s degree preferred
  • Minimum 2 years of experience in home care, senior services, or a related healthcare role
  • Prior experience in scheduling, case coordination, or caregiver supervision strongly preferred
  • Excellent verbal and written communication skills
  • Strong organizational skills with the ability to multitask and prioritize
  • Compassionate, empathetic, and committed to client-centered care
  • Proficient in Microsoft Office and scheduling software (ClearCare/WellSky a plus)
  • Reliable transportation and willingness to travel locally for field visits
