- Conduct home visits in assigned territory to assess home safety, gather changes in health status, and provide clinical coaching and support including review of medications
- Following an established care plan, provide ongoing coaching and support to caregivers via app, including resource referral, and reinforcement of best practices.
- Develop trusting relationships with caregivers to influence change and reduce caregiver burnout
- Document and review records for accuracy and timeliness, ensuring compliance with regulatory and accreditation standards
- Other projects and duties as assigned.
- Perform other duties as assigned.
- Must hold an active Registered Nurse (RN) License in the state of employment
- Minimum two years of recent experience in direct care of elders or adults with disabilities
- Ability to travel (usually by automobile) within assigned geographic area; valid driver’s license and automobile insurance is required
- Experience in case management, care planning and assessment for cases including but not limited to eldercare, people with disabilities, and other complex medical needs in a health care setting
- Proficiency in common software and web-based applications (e.g. MS Office, case management systems)
- Additional state-specific qualifications may be required.

The pay range for this position is $75,969 - $93,035. The actual wage offered may be lower or higher depending on budget and candidate experience, knowledge, skills, qualifications, and geographic location.
