Hospice Social Worker

Hospice experience is a MUST

Responsibilities:

• Provide Clinical Social Work to Hospice population.
• Complete initial and ongoing social evaluation of patient and family dynamics
• Participates in the coordination of services with other hospice team members and community resources as needed.
• Maintains and processes all documentation in compliance with agency standards.

Requirements:

• Home Care, Hospice or Bereavement counseling experience
• Valid Driver’s License, auto insurance and reliable transportation
• Compassion and integrity

Job Type: Full-time

Pay: $86,000.00 - $96,000.00 per year

Ability to Commute:

• Virginia Beach, VA 23466 (Required)

Ability to Relocate:

• Virginia Beach, VA 23466: Relocate before starting work (Required)
