Job Overview
We are seeking a dedicated and compassionate Service Coordinator to join our team. This role involves managing and coordinating services for households in low-income housing, ensuring they receive personalized support and resources. The ideal candidate will possess strong management skills, experience in program development, and a passion for working with people with diverse needs. The Service Coordinator will serve as a key liaison between clients, families, service providers, and community resources to promote positive outcomes and quality of life.
Responsibilities
- Develop, implement, and oversee individualized service plans tailored to each client's needs
- Coordinate and monitor service delivery to ensure compliance with program goals and regulatory standards
- Manage relationships with clients, families, service providers, and community organizations to facilitate effective communication and collaboration
- Conduct assessments of client needs and adjust services accordingly
- Maintain accurate documentation, case records, and program reports in accordance with organizational policies
- Supervise staff or volunteers involved in service delivery when applicable
- Facilitate training sessions or workshops related to behavioral health, social work practices, or program management as needed
- Ensure adherence to all relevant policies, procedures, and legal requirements
Requirements
- Bachelor’s degree in Social Work, Human Services, Psychology, or related field (Master’s preferred).
- 2 years of experience in resident services, case management, social work, or community-based programming.
- Strong relationship management skills to build trust with clients, families, and community partners
- Administrative experience including documentation, reporting, and budgeting skills
- Excellent organizational skills with attention to detail
- Compassionate approach with a commitment to supporting individuals’ independence and well-being
- Ability to handle sensitive situations with professionalism and confidentiality
Work Environment
- Based at Serve Denton with house visits made to scattered site properties made daily.
- Regular interaction with residents, property management staff, and community partners.
- May require occasional evening or weekend availability for resident events or emergencies.
