Job Overview
We are seeking a dedicated and compassionate Service Coordinator to join our team. This role involves managing and coordinating services for households in low-income housing, ensuring they receive personalized support and resources. The ideal candidate will possess strong management skills, experience in program development, and a passion for working with people with diverse needs. The Service Coordinator will serve as a key liaison between clients, families, service providers, and community resources to promote positive outcomes and quality of life.

Responsibilities

  • Develop, implement, and oversee individualized service plans tailored to each client's needs
  • Coordinate and monitor service delivery to ensure compliance with program goals and regulatory standards
  • Manage relationships with clients, families, service providers, and community organizations to facilitate effective communication and collaboration
  • Conduct assessments of client needs and adjust services accordingly
  • Maintain accurate documentation, case records, and program reports in accordance with organizational policies
  • Supervise staff or volunteers involved in service delivery when applicable
  • Facilitate training sessions or workshops related to behavioral health, social work practices, or program management as needed
  • Ensure adherence to all relevant policies, procedures, and legal requirements

Requirements

  • Bachelor’s degree in Social Work, Human Services, Psychology, or related field (Master’s preferred).
  • 2 years of experience in resident services, case management, social work, or community-based programming.
  • Strong relationship management skills to build trust with clients, families, and community partners
  • Administrative experience including documentation, reporting, and budgeting skills
  • Excellent organizational skills with attention to detail
  • Compassionate approach with a commitment to supporting individuals’ independence and well-being
  • Ability to handle sensitive situations with professionalism and confidentiality

Work Environment

  • Based at Serve Denton with house visits made to scattered site properties made daily.
  • Regular interaction with residents, property management staff, and community partners.
  • May require occasional evening or weekend availability for resident events or emergencies.

Job Type: Full-time

Pay: From $50,000.00 per year
