What You'll Do:

The Clinical Care Coordinator will support the care needs of those seeking or requiring treatment for behavioral health needs, particularly for priority populations such as individuals placed on an Involuntary Commitment (IC) due to substance use disorder or those experiencing complex or high acuity mental health needs. This role will support Signal’s commitment to ensuring the availability and access to high-quality behavioral health services in the regionally contracted counties on behalf of the Behavioral Health Administration (BHA) of the State of Colorado.

The Clinical Care Coordinator will serve as a vital member of the Clinical Services team and will help to monitor, plan, advocate, and link individuals to clinically appropriate services and resources.

  • Smooth the transition: Help individuals navigate crucial life changes with thoughtful coordination.
  • Forge partnerships: Collaborate with a wide range of community partners to build a strong support system.
  • Champion quality: Work closely with service providers to ensure the best possible care.
  • Innovate and improve: Contribute to making our programs even better.
  • Keep it organized: Document your impactful work clearly and efficiently.

What You'll Bring:

  • A Master's degree in a relevant field (Social Work, Counseling, Marriage and Family Therapy, etc.) and associated licensure/certification in Colorado (LCSW, LPC, LMFT, LAC, or CAS).
  • A passion for helping others and a strong commitment to our mission of combating substance abuse.
  • At least two years of experience in clinical service delivery, including assessment, treatment, and case management for individuals with substance use disorders.
  • Knowledge of practical and theoretical application of ASAM.
  • Previous supervisory or leadership experience is a plus!
  • Excellent communication, collaboration, and problem-solving skills.
