Givens Choice is looking for an experienced Care Manager who will be responsible for building and maintaining professional relationships with Members, their support networks, physicians, service providers and all other entities involved in Members care. In this role you will provide an individualized care plan for each Member of the program using effective and efficient geriatric care management techniques.
Schedule: M – F with rotating on call schedule.
What you’ll do:
- Implement and manage current operational processes and procedures in accordance with the standards and procedures set out by the Executive Director and Director of Member Services.
- Meet personally with prospective clients to answer questions about the program and perform assessment components of the membership application process.
- Manage aspects of relationships with credentialed vendors, including contracting, service provision, quality monitoring, and annual updates.
- Manage a caseload of members, including regular communication with Members, assessments of needs, coordinating and scheduling services.
- Develop and implement Member Wellness Plans, and Member Support Plans.
- Act as a Liaison between the Aging in Place program and Member caseload.
What you’ll need:
- 2 + years of social work experience in a retirement community
- 2+ years of Care Management experience
- Bachelor’s degree
- Valid Driver’s License
Compensation: $65,000 - $70,000
