Responsibilities:

• Conduct initial assessments and develop comprehensive care plans for residents, taking into account their physical, emotional, and social needs
• Collaborate with residents, families, and healthcare teams to identify and address concerns, while promoting independence and autonomy
• Develop and implement individualized treatment plans, utilizing evidence-based practices and interventions
• Provide counseling, support, and education to residents and their families, addressing mental health, grief, and bereavement issues
• Coordinate and facilitate interdisciplinary care conferences, ensuring effective communication and collaboration among team members
• Monitor and evaluate the effectiveness of care plans, making adjustments as needed
• Maintain accurate and thorough records, adhering to regulatory requirements and company policies

Requirements:

• Bachelor's degree in Social Work or related field (Master's degree preferred)
• Minimum 3 years of experience in social work or a related field
• Licensure as a Social Worker in the state of New Jersey (or ability to obtain within 6 months of hire)
• Strong communication and interpersonal skills, with the ability to work effectively with diverse populations
• Ability to maintain confidentiality and adhere to HIPAA guidelines
• Strong organizational and time management skills, with the ability to prioritize tasks and manage multiple responsibilities
