Position Overview

We are seeking an experienced Social Worker for a 13-week contract assignment in Fort Pierce, FL. This role supports patient care coordination, discharge planning, and psychosocial assessments within an acute care or clinical setting. The ideal candidate will be highly organized, patient-focused, and experienced with hospital-based social work workflows.

Key Responsibilities

• Conduct comprehensive psychosocial assessments and develop appropriate care plans
• Coordinate discharge planning and transitions of care
• Complete PASRR screenings and documentation as required
• Collaborate with interdisciplinary healthcare teams, including physicians, nurses, and case managers
• Utilize MIDAS, Meditech, and NaviHealth systems for documentation and care coordination
• Advocate for patients and families by connecting them with appropriate community resources
• Ensure compliance with hospital policies, regulatory standards, and patient confidentiality requirements

Required Qualifications

• Master of Social Work (MSW) or Licensed Clinical Social Worker (LCSW)
• Experience with MIDAS, Meditech, PASRR, and NaviHealth
• Strong knowledge of discharge planning and care coordination processes
• Excellent communication, documentation, and organizational skills
