The Medical Social Worker (MSW) assess the social and emotional factors related to a patient's illness, need for care, functional capacity, and ability to cope with the challenges of daily living. The MSW collaborates with the interdisciplinary team members in the development of individualized treatment plans and actively participates in case conferences and care planning meetings.
Status: Per Diem

Education/Experience/Skills/ Qualifications:
- Master's degree in Social Work from a program accredited by the Council on Social Work Education
- At least one (1) year of social work experience in a healthcare setting, preferably within a home health agency
- Current CPR certification
- Strong assessment, communication, and interdisciplinary collaboration skills
