Position Summary
The Palliative Care Coordinator is responsible for coordinating non-hospice palliative care services to ensure timely, patient-centered, and compliant delivery of care in accordance with CMS Conditions of Participation and CHAP standards.
This role supports the interdisciplinary team by facilitating communication, scheduling services, monitoring patient status through routine follow-up, and maintaining accurate documentation to promote continuity, quality, and compliance.
Essential Functions & Responsibilities
Care Coordination & Patient-Centered Services
- Coordinate palliative care services in collaboration with licensed clinicians and the interdisciplinary team (IDT).
- Schedule initial consultations, follow-up visits, and ancillary services in accordance with the established plan of care.
- Conduct routine patient wellness and follow-up calls to identify needs, concerns, changes in condition, or barriers to care.
- Communicate patient status changes promptly to the appropriate clinical staff for assessment and intervention.
- Support appropriate transitions of care, including coordination with home health or hospice services when clinically indicated and authorized.
Interdisciplinary Collaboration
- Serve as a liaison between patients, families, physicians, and the palliative care interdisciplinary team.
- Participate in care coordination discussions and IDT communication as assigned.
- Ensure patient and family communication is respectful, compassionate, and aligned with patient rights and preferences.
Documentation & Compliance
- Maintain accurate, timely, and complete documentation in the electronic medical record (EMR) in compliance with CMS and CHAP requirements.
- Track referrals, authorizations, scheduling, and follow-up activities to ensure timely service delivery.
- Maintain logs of patient contacts and coordination activities to support quality monitoring and audits.
- Adhere to HIPAA regulations, patient confidentiality standards, and agency policies and procedures.
Administrative Support
- Assist with intake coordination and information gathering as appropriate.
- Support agency processes related to quality improvement, compliance monitoring, and patient satisfaction.
- Follow established policies related to patient rights, complaint resolution, and service delivery.
Qualifications
Required
- Active LVN license or Medical Assistant (MA) certificate
- Minimum one (1) year of experience in a medical office, palliative care, hospice, home health, or comparable healthcare setting
- Demonstrated experience in:
- Care coordination and scheduling
- Patient follow-up and wellness calls
- Medical office operations and documentation
- Ability to work within interdisciplinary care models
- Strong organizational, communication, and time-management skills
- Proficiency with EMR systems and basic computer applications
Preferred
- Previous experience in palliative care or hospice
- Familiarity with CMS Conditions of Participation and accreditation standards
- Bilingual (English/Spanish) preferred
- Experience supporting quality improvement or compliance initiatives

Pay: $24.36 - $29.33 per hour
