Shift: Full Time, 9 AM - 5 PM

We are seeking an experienced and empathetic Social Worker to join our team in Brooklyn and help support residents and their families with care, guidance, and advocacy.
What You’ll Do
- Conduct resident and family interviews to develop detailed social histories and individualized care plans.
- Work closely with families to design inclusive programs that promote residents’ emotional, social, and psychological well-being.
- Provide guidance and solutions for challenges such as financial resources, discharge planning, and community support services.
- Collaborate with an interdisciplinary team—including administration, nursing, and medical staff—to ensure holistic and coordinated care.
- Advocate for residents by addressing their social and emotional needs while maintaining clear communication across the care team.
- Minimum of 2 years of experience as a Social Worker in a Skilled Nursing Facility (SNF) required.
- Strong interpersonal and communication skills, with the ability to build trust with residents, families, and colleagues.
