Lead with purpose. Create impact. Sustainable Wellness Solutions is a peer-run nonprofit that supports individuals living with mental health and substance use challenges. We are hiring a Case Manager, Generalist to provide direct services across housing, health, and stability supports for Sacramento residents, including people experiencing homelessness and those living with chronic illness or disability.


What You Will Do

  • Deliver client-centered case management, including intake, assessment, goal planning, and ongoing support
  • Coordinate housing navigation, benefits linkage, transportation, and referrals to medical, behavioral health, and social services
  • Maintain accurate and timely documentation, case notes, and outcome tracking in HMIS and agency systems
  • Advocate for participant needs with county partners, hospitals, shelters, and community providers
  • Conduct field visits, home visits, and community-based outreach to meet clients where they are
  • Support warm handoffs and care transitions to ensure continuity and safety
  • Uphold SWS values of dignity, equity, and respect in every interaction

What We Are Looking For

  • 2+ years of experience in mental health, homelessness services, reentry, or related social services
  • Strong communication, problem solving, and boundary setting skills
  • Experience with Microsoft Office and cloud-based case management tools. HMIS familiarity preferred
  • DOJ background check required
  • Valid California driver’s license and reliable transportation for field work
  • Lived experience is valued and welcomed
