The Opportunity
We are seeking aforward-thinking, collaborative, and systems-minded psychiatrist leader to serve as Medical Director for multiple residential and outpatient sites serving individuals with mental health, substance use, and co-occurring conditions.
This is not a passive oversight role. The Medical Director will help design and steward Terra’s medical and psychiatric model of care, ensuring clinical excellence, regulatory compliance, and thoughtful integration between medical, nursing, and therapeutic services.
You will partner closely with Operations, Clinical leadership, Nursing, and Outreach to build programs that are safe, ethical, evidence-based, and human-centered — from day one.

What You’ll Do
Medical & Psychiatric Oversight:
- Provide medical and psychiatric leadership across two residential treatment programs and one outpatient site including all LOC from detox to outpatient.
- Establish and oversee medical policies, protocols, and standards of care.
- Ensure safe, evidence-based prescribing practices and medication management.
- Support assessment, diagnosis, and treatment planning for complex and co-occurring presentations
Clinical Quality & Governance
- Serve as the physician leader responsible for medical quality, safety, and clinical integrity.
- Participate in case reviews, high-risk case consultation, and clinical decision support.
- Guide clinical standards related to detox, withdrawal management, psychiatric stabilization, and medical comorbidities.
Interdisciplinary Leadership & Collaboration
- Partner closely with Nursing, Therapy, and Operations leadership to ensure integrated care.
- Support and supervise prescribing providers (e.g., Psychiatric NPs/PAs) and medical staff (RN, LVN’s) as applicable.
- Participate in leadership meetings and contribute medical perspective to program development and growth.
Compliance & Regulatory Oversight
- Ensure medical services comply with DHCS, Joint Commission, and applicable state and federal regulations.
- Support audits, licensing surveys, and accreditation readiness.
- Review incidents, adverse events, and medical risk concerns, providing guidance and corrective recommendations
Crisis & Risk Support
- Serve as a medical escalation resource for complex clinical situations or emergencies.
- Be available for 24/7 consultation during critical incidents, including medication-related or medical safety concerns.
Program Development & Growth
- Contribute to the design of new programs, services, and levels of care as Terra expands.
- Help define Terra’s medical philosophy and clinical differentiation in the market.
What You Bring
- ABPN Board-Certified MD Psychiatrist with active, unrestricted CA medical license
- Board certified with ABAM and member of ASAM (preferred)
- 2+ years of leadership in behavioral health, residential treatment, or multi-site healthcare organization (preferred)
- Strong financial acumen and comfort overseeing budgets and staffing models
- A builder’s mindset: resourceful, decisive, hands-on, and comfortable operating in ambiguity
- Outstanding communication, crisis management, and clinical judgement
- Experience launching new programs or opening new facilities (highly preferred)

Pay: $51,496.21 - $78,016.95 per year
