POSITION SUMMARY

Primary purpose of organizational unit is to provide public health services for Harnett County residents under the authority and guidance of the Harnett County Board of Health to establish policies and procedures for the various programs using laws, rules, regulations and guidance from federal, state and local governments.

Minimum Salary: $54,216.00
Job Rate: $63,027.00

DUTIES AND RESPONSIBILITIES

This position manages a caseload of assigned clients participating in the Medicaid funded CAP/DA program designed to keep individuals at risk of needing nursing home placement in their own homes. Conducts home visits and makes assessment of client's mental health, home environment, social support network, financial status, abilities and needs and writes or revises the Plan of Care.

MINIMUM QUALIFICATIONS

Bachelor's degree in social work from an accredited school of social work; Bachelor's degree in a human services field from an accredited college or university and one year directly related experience; Bachelor's degree from an accredited college or university and two years directly related experience,or an equivalent combination of education and experience.
