Welltel is launching an exciting new behavioral health program to serve our diverse patient population. We're seeking a pioneering Licensed Master Social Worker who's ready to build something meaningful from the ground up. What You'll Be Doing

• Provide therapeutic services via telehealth to patients at our FQHC locations

• Collaborate directly with our leadership to develop program protocols, screening processes, and workflows

• Conduct initial patient assessments and ongoing therapy sessions

• Help establish best practices for behavioral health integration within our primary care model

• Work with our team to identify and engage patients who would benefit from behavioral health support

The Ideal Candidate

Must Have:

• Licensed Master Social Worker (LMSW) in New York State

• Experience providing therapy and behavioral health services

• Comfort and proficiency with telehealth platforms

• Understanding of FQHC patient populations and their unique needs
