Job Summary
We are seeking a compassionate and proactive Resident Care Coordinator to join our dedicated team in providing exceptional care to residents in our assisted living . In this vital role, you will serve as a key point of contact for residents, families, and staff, ensuring personalized care plans are implemented effectively. Your energetic approach will help foster a warm, safe, and supportive environment where residents with diverse needs—including those with Alzheimer's, dementia. Responsibilities
- General management responsibilities include, but are not limited to the following:
- Responsible for fulfilling daily, weekly and monthly job checklist duties.
- Maintain current task lists for the Resident Assistants
- Assure all miscellaneous evaluations are completed in conjunction with the service planning process; this includes but is not limited to Safety and Smoking
- Become proficient in the service plan program, gathering the necessary information on the care needs of the resident prior to the service plan meeting and then being prepared to present any changes to the service plan to the resident and family members
- Implement interim service plans as care needs change and assuring all health service staff is aware of the changes
- Responsible for the over-site of ordering incontinent supplies and any durable medical equipment as needed
- Assure home health and hospice providers are communicating their visits and
- leaving information on their clients
- 8. Complete audits as directed by the Executive Director or Director of Resident
- Services
- 9. Assist Director of Resident Services with the tracking of incident reports and
- investigations
- General record keeping and filing as needed
- Maintain budget and spend down documentation
- Ensure that staffing levels remain within established and approved budgetary and staffing guidelines
- At times, fill in as a Medication Assistant and Resident Assistant
- Conduct yourself and the business at all times so as not to detract from, or to reflect adversely on the reputation of the Community
- Treat as confidential any information about the customers and personnel of the Community or their business, products, techniques, methods, systems, price books, advertising, plans and policies. Employee will not, during their employment, or at any time thereafter, disclose such information, in whole or in part, to any person, firm, or corporation for any reason or purpose whatsoever as stated in the signed confidentiality agreement.

Pay: $25.00 - $27.00 per hour
