Key Responsibilities
As a Care Coordinator, you will play a critical role in leading and supporting a team of caregivers and companions, ensuring that our clients receive the highest level of care. Your responsibilities will include:
- Leading and supporting a team of caregivers and companions, fostering a collaborative and compassionate environment.
- Overseeing the interviewing, hiring, and onboarding process for caregivers and companions, ensuring that we maintain a team of highly qualified professionals.
- Ensuring client satisfaction by maintaining high Net Promoter Scores and addressing any concerns promptly and effectively.
- Providing regular, constructive feedback to caregivers and companions, helping them grow and excel in their roles.
- Holding caregivers and companions accountable to high standards and company goals, ensuring consistency in care delivery.
- Managing the client database, ensuring that all records are current and accurate, including care plans, demographic information, and schedules.
- Overseeing staffing and scheduling to ensure that clients receive care from consistent caregivers or companions.
- Maintaining compliance with Department of Health and Human Services regulations for home care.
- Proactively working to increase client billable hours through effective care management.
- Conducting client visits and follow-up calls to ensure ongoing satisfaction and address any changes in care needs.
- Participating in the on-call rotation to provide support as needed.
Why You’ll Be a Great Fit
- Preferred: Bachelor’s degree in Social Work, Geriatric Care Management, or a related field.
- Preferred: Two to three years of experience in case management or a related field, particularly in geriatric care.
- Preferred: Two to three years of experience in managing a team, with a proven ability to provide feedback and hold team members accountable.
- Demonstrated success in building strong relationships that contribute to achieving organizational and revenue goals.
- Strong organizational skills with the ability to manage multiple priorities and deadlines.
- Excellent communication and interpersonal skills, with a focus on active listening and effective problem-solving.
- Proficiency in using online scheduling systems and maintaining accurate records.

Qualifications & Requirements
- Minimum of two years of experience in a similar role within home care or geriatric care management.
- Strong organizational and communication skills, with a proven ability to build relationships that drive organizational success.

Job Type: Full-time
Pay: $50,000.00 per year
