Medical Social Worker (MSW) - Home Health / Hospice

We are seeking a passionate Medical Social Worker (MSW) to work in our Hospice/Home Health departments! The role would include case managing patients in the Sacramento area.

Medical Social Worker (MSW) Responsibilities:

• Performs comprehensive health assessment of the client’s psycho-social, functional, and environmental status.
• Assesses the client’s informal (family/friends) and formal (community and institutional) support system.
• Carries out analysis and synthesis of all data to formulate appropriate social and interdisciplinary problem statements.
• Assures that all clients are aware of rights and responsibilities.
• Assures that all information concerning client is treated confidentially.
• Development, implementation, assessment, and modification of an individualized plan of care under a physician’s order.
• Assures that an individualized and goal directed plan of care, including safety issues, are

developed and implemented for the client.

• Assures that physician orders for all visits and follow ups are obtained on a timely basis and in
• accordance with applicable laws and regulations.
• Participates in team conferences and assures accurate documentation regarding clients
• discussed.
• Modifies care plan as necessary based on client status and physician direction.
• Implementation of the plan to ensure the quality, quantity, timeliness, and effectiveness of services are provided.
• Meets with client for the purpose of admission and development of care plan; conflict resolution; benefits and entitlement counseling, and community resource referral.
• Links the client with the most appropriate and cost effective community and/or institutional resources, advocating on behalf of the client for resources.
• Coordinates care plans, goals, and visit schedules with other team members and service providers.
• Maintains effective communication with client, physician, team members, service providers, supervisor, and others involved in providing client care.
• Maintains an accurate documentation record that reflects care/service provided.
• Follows standard documentation format for documenting care/service provided.
• Assures accurate and timely completion of data as required.
• Utilizes community resources to assist client to maintain optimal level of functioning

Medical Social Worker (MSW) Education/Training:

• Completion of a Social Work degree from a school of social work accredited or approved by the Council on Social Work Education is required.

Medical Social Worker (MSW) Experience:

• A minimum of one year experience in home health care services
• Working knowledge of and identification with casework philosophy, principles, practice, and community resource utilization is required. Working knowledge of benefits programs, community programs resources and services.

Days/Hours:

• We operate Monday through Sunday
• MSWs set their visits with their clients allowing for a flexible schedule
