Kingsway Arms Nursing Center is currently seeking a Social Worker.
The Social Worker maintains the social and material needs of the residents in the facility. He/She acts as a liaison between the resident, family and community as indicated. He/She is knowledgeable of other agencies and organizations both public and private, which provide for the overall needs of the residents. The social worker fulfills an integral part of the interdisciplinary care team. He/she must have good organizational skills and the ability to communicate effectively. The Social Worker uses problem solving skills as well as evaluation, counselling and referral when appropriate.

Requirements
Principal Responsibilities/Duties
- Know facility policies and procedures and fire/disaster plan
- Attend all required in services and meetings
- Maintain communication with all disciplines so each resident’s plan of care is comprehensive
- Maintain communication with resident designated representative as indicated
- Maintain firm adherence to the Resident Bill of Rights and strict confidentiality concerning all aspects of resident’s care
- Act as discharge planning coordinator
- Receive complaints/concerns from resident or responsible party and/or staff and communicate with team to resolve and follow up
- Conduct psychosocial assessment/social history
- Review and complete MDS questions and assessments as indicated
- Assist resident and family with adjustment and transition to facility
- Participate in resident care plan meetings
- Provide resident referral for psychiatry or psychology services as indicated
- Document resident psychosocial status initial and ongoing team notes, goals and progress
- Coordination of resident discharge planning and follow up with resident, family, interdisciplinary team and community services
- Complete resident care plans as indicated
- Review, educate and assist in completion of advance directives with resident and or designated representative
- Assist with end of life care planning
- Participate in resident and family council as requested
- Facilitate new employee orientation as needed
