AOM Healthcare is a premier long term care company, operating twenty one long term care facilities throughout the state of Ohio and is now hiring Full-Time Social Services Director at Concord Care at Hartford located at 3090 Five Points Hartford Rd., Fowler, Ohio.

Job Duties & Requirements:

Qualifications: Possesses a Master’s Degree from an accredited institution.

One to two years of experience in a related field preferred.

Possesses growth in the profession through community involvement and continuing education.

Explores and utilizes available community resources applicable to residential health care facility programs.

Must meet health assessment requirements, including two stage Mantoux skin test.

Must be capable of performing the essential functions of the job, with or without reasonable accommodation.

Must be able to communicate in English, both verbally and in writing, and possess sufficient communication skills to perform the tasks required.

• Job Type : Full Time (Salary) negotiable

• Experience : Long term care management (Preferred)

• Pay will be based on experience.

The candidate must be able to communicate well with staff, physicians, residents, and visitors. Candidate attributes include but not limited to:

• Able to be a leader and team player

• Able to follow state regulations and policies of facility.
