ClarityACP offers Medicare-reimbursed advance care planning services via telehealth, enabling patients to clarify their healthcare preferences and goals. Founded by a physician, we partner with healthcare providers across St. Louis to ensure patients have meaningful conversations about their future care.

The Role

Conduct compassionate, professional advance care planning conversations via telehealth with Medicare patients. You'll help individuals and families navigate complex healthcare decisions, document their wishes, and ensure their voices are heard in their medical care.

What You'll Do

• Conduct 30-45 minute telehealth ACP sessions with Medicare patients
• Guide patients through healthcare decision-making, goals of care, and advance directive completion
• Navigate family dynamics and facilitate difficult end-of-life conversations with empathy
• Complete thorough session documentation within 48 hours
• Coordinate with referring healthcare providers as needed

Who You Are

• Licensed Clinical Social Worker (LCSW) in Missouri
• Experience in hospice, palliative care, or geriatric settings conducting ACP conversations
• Comfortable with telehealth technology and video platforms
• Strong documentation skills and attention to detail
• Maintain professional liability insurance (NASW membership or equivalent)
• Available for part-time, flexible scheduling (5-15 sessions/week initially, growing to 20-30)

Compensation & Schedule

• $30 per completed session
• Compensation increases as ClarityACP scales and partnerships expand
• Work from home via telehealth
• Flexible scheduling - you control your availability
• Part-time contractor position (1099)
• Average 30-45 minute sessions
