Case Manager/Discharge Planner for DMH Medi-Cal residential mental health program

We are a 16 bed L.A. county Medi-Cal DMH mental health residential treatment program. Our clientele typically presents with diagnoses such as schizophrenia, depression, anxiety, delusional disorders, and bipolar disorder. All of our patients are referrals from hospitals. Many are homeless. Our ideal candidate for this job is someone who has extensive knowledge of and experience accessing L.A. county resources for our clients such as housing (FSP, ERS, etc)

Must have previously passed a LiveScan federal fingerprint background check or be able to pass one upon hire.

Case management duties: Helping our clients to obtain housing, disability paperwork, appointments, and discharge planning. You will be helping our homeless clients find housing options. You will be documenting heavily and using our EMR system

Requirements: Must have experience in case management, preferably with Los Angeles county Medi-Cal population. Must live in Los Angeles County. This job is fully in person and not remote.

Experience

• Extensive knowledge and resources in Los Angeles County DMH (i.e. Full-service Partnerships, ERS, IMD)
• Discharge planning experience required
• Case management experience required
• Los Angeles County DMH experience preferred but not required
• Mental Health experience a plus

Job Type: Full-time

Pay: $21.00 - $25.00 per hour
