Licensed Clinical Social Worker (LCSW) – Home Health

About Us

K&N Rehab Inc. is seeking skilled and compassionate Licensed Clinical Social Workers (LCSWs) to join our home health team.

Coverage Area: We serve the entire Santa Clarita but not limited to:

• Santa Clarita
• Valencia
• Saugus
• Newhall
• Canyon Country
• Stevenson Ranch
• Castaic

We offer flexibility to work within your preferred areas.

Key Responsibilities (for LCSW)

• Conduct psychosocial assessments for patients in the home health setting.
• Provide counseling and emotional support to patients and families coping with illness, disability, or life changes.
• Develop and implement individualized care plans addressing social, emotional, and resource needs.
• Assist patients and families in accessing community resources, support services, and financial/insurance benefits.
• Collaborate with the interdisciplinary healthcare team to ensure holistic and coordinated patient care.
• Provide crisis intervention when necessary.
• Maintain accurate, timely, and compliant documentation.

Requirements (for LCSW)

• Current California Licensed Clinical Social Worker (LCSW) License (in good standing)
• Master’s degree in Social Work (MSW) from an accredited program
• EIN (Employer Identification Number) or willingness to obtain one (for independent contractor work)
• Valid CA Driver’s License and reliable transportation
• Auto insurance
• Current CPR Certification
• Up-to-date Physical Exam & TB test
• Strong communication, counseling, and organizational skills
• Ability to work independently and manage a flexible schedule
• Home health or healthcare experience preferred, but not required
