POSITION TITLE: In-Home Outreach Counselor, Family Preservation Program
DUTIES AND RESPONSIBILITIES:
Under the supervision of the Family Preservation Program Manager, the In-Home Outreach Counselor will:
- Conducts comprehensive intake and assessment with each family to evaluate specific needs and coordinate all systems of care.
- Complete assessments every 90 days.
- Provide one-hour, in-home counseling and clinical case management sessions to families in and around Service Area 7 & Service Area 8 (LA County Cities: Santa Fe Springs, Commerce, Long Beach and surrounding Communities) once per week, at minimum.
- Conduct unannounced visits and child follow-up sessions, as needed, to ensure child safety.
- Schedule a Multidisciplinary Case Planning Committee (MCPC) Meeting every 60 days with parents, FP manager, and Children’s Social Worker (CSW) for each family.
- Create and develop goals for each family by completing an MCPC service plan for every MCPC meeting.
- Provide appropriate resources/referrals to families to assist with court mandates and/or assist with each family’s needs, as well as follow-up on progress.
- Collaborate and maintain effective communication with the assigned CSW to assist in accomplishing case plan goals and outcomes for families.
- Complete and submit monthly reports for all cases to the assigned CSW on the case.
- Maintain an average caseload of approximately 10-12 families per week.
- Maintain accurate and timely documentation of all job-related activities including weekly progress notes, assessments, monthly reports, treatment plans, within 3 business days.
- Attend weekly individual and group supervision, as well as individual and team program meetings.
- Conduct one-time Family Preservation Assessment Services, with parents involved with DCFS, by conducting a 2-hour Biopsychosocial Assessment and complete and submit a summary report with diagnosis and recommendation for treatment within 72 hours.
- Other duties as assigned.
REQUIREMENTS:
- Master’s Degree (Preferred) or Bachelor’s Degree required in Arts/Science in social services, Social Work, Human Services, Counseling, Criminal Justice or Child Development or related field with 6 months – 1 year of case management experience.
- Bilingual in English and Spanish
- Ability to work a flexible schedule, including evenings and weekends, as needed.
- Permanent U.S. residency or citizenship.
- Vehicle for use to conduct in-home sessions.
- Valid California Driver’s license, current auto insurance, and good driving record.
- Ability to pass background check and criminal clearance.
- Experience in working in a community-based organization.
- Knowledge of Southeast Los Angeles County.
