BILINGUAL HOSPICE SOCIAL WORKER (MSW)

Based out of our Torrance office (with excellent parking) we are dedicated to providing exceptional end-of-life care.

About the Role
As a Hospice Social Worker, you’ll partner with patients, families, and the interdisciplinary team to support the emotional, social, and practical needs associated with serious illness and end-of-life care.
Your responsibilities will include:
- Conducting psychosocial assessments and developing individualized care plans
- Providing emotional support, counseling, and crisis intervention
- Helping families process change, grief, and complex decisions
- Assisting with advance care planning and end-of-life discussions
- Coordinating community resources, financial support options, respite services, and additional needed support
- Advocating for patient rights, preferences, and goals
- Supporting interdisciplinary team collaboration to ensure whole-person care

Qualifications
- Master’s degree in Social Work from an accredited program.
- Minimum of 1 year in healthcare (hospice experience preferred).
- Excellent verbal, written, and organizational abilities.
- Current CPR certification required.
- Bilingual in Spanish preferred.
- Deep understanding of hospice philosophy and sensitivity to issues surrounding death and dying.
