Position Summary
Job Summary
Under the direction of the Community Connect Program Supervisor, the ECM Care Manager delivers direct services and care coordination as part of a multidisciplinary team. You will assess whole-person needs, develop and execute care plans, advocate for clients, and ensure timely access to health and social services that drive wellness, recovery, independence, and empowerment.
Essential Job Responsibilities

Assess client needs across physical health, mental health, SUD, oral health, trauma history, social supports, housing, employment, and community services.

Develop and oversee Individual Care Plans (ICP) and Health Action Plans (HAP).

Provide services in-office, via telehealth, and in the field—meeting clients where they are.

Connect clients to community resources and social supports; adjust referral processes for cultural relevance.

Coordinate care with providers (with consent); maintain continuity from assessment through discharge.

Apply evidence-based approaches (Motivational Interviewing, Trauma-Informed Care, Harm Reduction).

Collect, organize, and interpret behavioral health and psychosocial data.

Document services clearly, timely, and compliantly in the EHR.

Attend required program meetings/trainings; uphold professional conduct and HIPAA.

Perform additional duties as assigned.
Essential Job Requirements

Prior experience in care coordination or case management (required).

High school diploma or equivalent (required).

Familiarity with local community resources and support services.

Strong interpersonal skills, empathy, active listening, sound judgment, and teamwork.

Proficient in Microsoft Office, Office 365, Teams, Zoom; comfortable with EHRs.

Valid CA driver’s license; insured, reliable transportation (agency authorization required).

Professional, courteous relationships with staff, vendors, and partners.
Preferred Qualifications

Associate’s/Bachelor’s in Psychology, Social Work, Public Health, Communications, or related field.

Experience in mental health or social services.
-
Bilingual English/Spanish (preferred, not required).
