POSITION SUMMARY: Field Care Navigator provides Care Management to patients in a non-clinical setting. Bringing together the clinical needs and social determinants of health to create a comprehensive care plan that serves the whole person. This position requires the ability to travel between connected counties and serve patients in person and remotely within those counties.

Responsibilities

  • Outreach
  • Client intakes
  • In-person client assessments- must be willing to travel between connected counties that we are contracted for.
  • Provide immediate support to identified and connected patients
  • Develop care plans with patients that address their immediate, short term and long-term goals for their health, housing, food, transportation, medical care, mental health care, finances, safety, and other social determinants of health
  • Care plan support through goal setting and assistance with achieving goals of the care plan
  • Collaborate with multiple community agencies and organizations to meet patients’ needs.
  • Accompany and advocate with social service agencies, as appropriate.
  • Maintain accurate client and other agency records in a timely manner.
  • Represent the agency at community meetings/functions through participation in collaborative meetings and community outreach presentations.
  • Perform other duties, as assigned.

• Proficient with technology, software and computer and other tech equipment as necessary to operate patient management software, other necessary programs, and equipment required to deliver all Master•Care Services

  • Punctual, organized, and efficient
  • Licensed Driver that is willing to travel to connected counties
  • Degree or other credentialing in one or more of the following: Social Work, Housing Navigation, Homeless Services, Geriatric Care Management or equivalent verifiable professional work experience
  • Three or more years of quality assurance, delivery, and management experience in health care, homeless services, community-based services, senior living and/or care, or similar environment
  • Knowledge of and experience with both clinical and non-medical services for a diverse population including the elderly, homeless, and other adults experiencing medical and social challenges.

• Ability to communicate professionally and effectively with patients, families, providers, community resources, and all Master•Care team members.

  • Bilingual or multilingual desired but not required
  • The ability to perform the physical demands of this position including but not limited to:

1. Sit and/or stand for long periods of time

2. Navigate stairs, bend, and reach

3. Lift, push, or pull a minimum of 10 lbs.
